How to make a complaint

You can only make a complaint to us if the issue affected you. |
|
|
You can also choose someone you trust to make a complaint for you. We call this person your representative. |
|
|
For example:

We will check if you agree to have your representative make a complaint for you. |
|
|
You representative will also need to give us proof that they can speak for you. For example, a letter that:

|
|
You need to contact us to explain your complaint clearly. |
|
|
You should tell us what you have already done to try to fix the issue. |
|
You should include: |
|
|
|
your contact details |
|
|
your provider’s contact details |
|
|
the names of other people you have contacted. |
|
You should also include: |
|
|
|
important dates, like the date when the issue started |
|
|
copies of letters and documents. |
|
|
You also need to tell us what you want to happen after you make a complaint. |
