Using AI
If you used artificial intelligence (AI) to help write your complaint, please say so in the form. Please check the details yourself, especially if the AI was used to refer to legislation or previous cases.

Length of complaints
Please limit your complaint to no more than three pages (3) or 1,000 words. This helps us to review your complaint and respond as quickly as possible. We will ask for more information if we need it.
Before you send your complaint to us
We can only accept complaints about health service providers in Tasmania.
If you are not sure if you can make a complaint to us, use this simple decision tree to find out whether we can help you.
Before making a complaint to us, please raise your issue with the health service provider concerned and try to resolve it directly with them. Put your concerns in writing and give the provider a chance to respond. If you are not satisfied with the provider's answer, or if there is a long delay, you can complain to us.
