How to make a complaint
Normally we would expect you to try to resolve your complaint with the department or authority concerned in the first instance. You can do this either by writing to them or by telephoning them to discuss your complaint with a departmental representative. If you are unable to resolve the matter to your satisfaction, you may complain to the Ombudsman.
To lodge a complaint with the Ombudsman you must submit your complaint in writing. Use this complaint form or contact us for a copy. You should set out your complaint in writing as simply as possible and tell us what you hope to achieve as an outcome to your complaint. You may wish to include:
- dates, times and locations of events
- what happened
- to whom you have spoken (names, position in the organisation and dates)
- copies of letters or documents relevant to your complaint.
